The three award-winning case studies explore their work to improve screening rates for patients with conditions such as diabetic retinopathy and autism detection in youth.

1. Diabetic Retinopathy Screening in the Primary Care Setting
El Rio Community Health Center recognized patient adherence to diabetic retinopathy screening was about 30%. The organization worked to address this by embracing technology that would allow specialty services to become integrated into the primary care workflow to improve health outcomes and quality measures for people living with diabetes. The organization explored retinal screening cameras, considering factors such as quality, size, price and ease of use. They also leveraged analytics to determine the best locations for the cameras. As a result of their work, over 3,000 patients with diabetes had increased options for completing the screening.
2. Autism Screenings for 24 Month-Olds
To improve autism screening rates in toddlers at El Rio Health, clinical and IT teams partnered together to create reports identifying care gaps for children needing well visits. Using the electronic health record to analyze and monitor autism screenings allowed teams to monitor their progress, leading to the attainment of screenings over 80% across the organization. Over the past two years, over 100 children have screened positive with 22 children who eventually were formally diagnosed with autism and are now receiving active interventions.
